Understanding and Managing Sciatica
Sciatica is one of those conditions that tends to announce itself loudly. Whether it's a sharp, electric pain shooting down the back of the leg, a deep burning ache in the buttock, or tingling and numbness that creeps into the foot, sciatic nerve pain has a way of making itself impossible to ignore.
The term “sciatica” refers to pain that travels along the path of the sciatic nerve — the longest nerve in the body, running from the lower spine, through the buttock, and down each leg. When something compresses or irritates the nerve — or one of the nerve roots that feed into it — the result is the distinctive leg pain that most people recognise as sciatica.
Common signs and symptoms
- Pain that starts in the lower back or buttock and travels down one leg
- Burning, shooting, or electric-like pain along the leg
- Tingling or pins and needles in the leg, calf, or foot
- Numbness or weakness in the leg or foot
- Pain that worsens when sitting, coughing, sneezing, or straining
- Difficulty standing or walking for extended periods
- Muscle cramping in the leg or buttock
What causes sciatic nerve irritation?
The most common causes involve something placing pressure on one of the spinal nerve roots that form the sciatic nerve — most often in the lower lumbar spine. A bulging or herniated disc is a frequent culprit, but joint restriction or inflammation in the lower lumbar or sacroiliac joints, tight muscles around the hip (particularly the piriformis), and postural load can all contribute.
Identifying the cause is important, because it guides the type of care that's most appropriate — and what to avoid.
How chiropractic care may help
Dr Renae carefully assesses the lumbar spine, sacroiliac joints, pelvis, and surrounding musculature to identify what's driving the nerve irritation. She'll also perform orthopaedic and neurological tests to understand the pattern and severity of the sciatic involvement.
Where chiropractic care is appropriate, she uses gentle techniques to address joint restriction and muscle tension that may be loading the nerve. Care is always adapted to the stage of the condition — acute sciatica requires a different approach than a long-standing, more chronic pattern.
She will advise clearly on what she can and can't help with, and will refer you to your GP, radiologist, or specialist where further investigation or a different approach is needed — particularly if symptoms include bladder or bowel changes, which require urgent medical attention.

For many people with sciatica, yes — particularly where the nerve irritation relates to lumbar joint restriction, tight muscles around the hip, or sacroiliac dysfunction. Dr Renae assesses the full picture before beginning care and will tell you honestly whether chiropractic is appropriate for your type of sciatica, or whether imaging or a specialist referral is the better first step.
Most sciatica resolves with appropriate conservative care over weeks to months. However, some presentations require urgent attention — particularly if you notice any changes to bladder or bowel function, significant weakness in the leg, or progressive numbness. If you experience any of these symptoms, please seek emergency medical assessment rather than waiting for a chiropractic appointment.
Prolonged sitting is one of the most common aggravating factors — particularly if the sciatic nerve is being compressed by a disc or tight muscle while the hip is flexed. Coughing, sneezing, and straining can also temporarily increase pain. Dr Renae will advise on positions and activities to modify during your recovery.
No — you don't need imaging before your first appointment. If you have existing scans, bring them along. Based on her assessment, Dr Renae will advise whether imaging would be useful and can guide you on the appropriate referral if needed.
